What is the Aim of This Survey?
This survey seeks to support awareness raising efforts to bring attention to the work conducted by the Indiana Balance of State continuum of care (BoS CoC) network through highlighting success stories gathered from across the network.
What Information Are We Collecting?
To help improve public understanding the Indiana BoS CoC work we are collecting success stories from providers within the network as well as those who have previously received services via the network. This information will be collected via this survey or through a follow-up interview that will be short, between 15-30 minutes.
How Will We Use Information Shared?
The information you share with us will help inform the creation of success story pieces to highlight the work conducted by those within the Indiana Balance of State continuum of care (BoS CoC) network. Success stories will be written up (no audio recording shared) and shared with IHCDA staff and the BoS CoC to share in public facing pieces such as through newsletters or via social media. You can opt-in or not of having your name included in these promotional items. If you complete this survey, you are consenting to the sharing of the information you provide for these efforts.
