Instructions and Contact Information

Organizations are invited to complete the application, based on the role(s) they wish to fulfill to implement IMPACT Legacy at the regional and local levels.  Application sections represent the following roles:

·         Section A: Fiscal Lead Agency
·         Section B: Regional Hub Lead Agency
·         Section C: Local Consortium Lead Agency

Applications must be submitted and received by 5 p.m. Pacific Time on June 12, 2023 at 5:00 p.m. If your organization is unable to complete the application in one sitting, users may return to the same computer to complete the application.
