Community Needs Assessment for People with Disabilities 2018
Thank you in advance for your voluntary participation in this survey to help assess the community needs for people with disabilties. The results of the survey will help Independent Living Resource Center, Inc. (ILRC) evaluate current programs and services; as well as, assist in planning and prioritizing for our future programs and services in our service area in these Central Missouri Counties: South Callaway, Camden, Cole, Miller, Moniteau, Morgan and Osage.
ILRC is a non-residential, consumer controlled, not for profit organization that provides services and support for individuals with disabilities through various programs and services. Promoting greater independence and empowerment for persons with disabilities is our mission here at ILRC.
The information collected is anonymous and your identity and participation will remain confidential. You are under no obligation to take this survey, but your input is appreciated. The survey will take approximately five minutes to complete.
