The Indigenous Diabetes Health Circle (IDHC) is asking our partners, communities, and clients for their thoughts and ideas. We want to understand how our communications, engagement, and messages are currently received and how we can improve communications as our organization grows and changes. 

This survey will take approximately 5-10 minutes. It is available until Friday, March 29, 2019, at 5 pm EDT. 

The survey is open to anyone who interacts with IDHC--our partner organizations, communities in which we work, healthcare professionals, clients, and anyone interested in diabetes services for Indigenous peoples.

Your opinions matter! Completing this survey is an important way for you to have your voice heard and contribute to shaping the future of IDHC.

Rest assured, your privacy will be protected. This survey is anonymous and is being conducted by Laridae, a consulting firm that specializes in non-profit organizations. Survey results will be provided to IDHC as grouped data. Individuals cannot be identified. It is possible that excerpts from your anonymous comments may be included in a report that summarizes the survey results; however, any potentially identifying information will be masked or removed.
