Survey Overview

Thank you for participating on the ICLAC Partnership Survey (IPS).   You received this survey based on a request that ICLAC's partnering organizations forward the survey to other key Los Angeles County stakeholders serving communities at high risk for vaccine preventable diseases (VPD). The survey is intended to better understand your organization's vaccine information resource needs by vaccine and population type.  The survey data will inform the development of a culturally and linguistically sensitive vaccine communication toolkit to help providers answer questions about the medically recommended vaccines.   
 
This is not a test.  Please answer the questions to the best of your knowledge.
 
Participation is voluntary and your responses will be kept confidential.  All results will be summarized and no individual responses or contact information will be shared outside the project team.  In appreciation for your time, successfully completing the survey will entitle you to a $10.00 Target gift card (only non-LA County staff).
 
Your involvement and the information you share will not affect you or your organization's relationship with ICLAC.

The survey has a total of 34 straightforward questions and should take you no longer than 10 minutes.  Please complete the survey no later than November 15, 2018.
