This survey is designed to show your District’s intent to implement an industry credential only program during FY2019 (2018-2019) consisting of the recently approved industry credentials added to the 2018-2019 school year approved career field list.

This form is not for Program Approval.  Once the FY2020 CTE-26 application window opens November 1, 2018, your district will need to complete the full CTE-26 application and process.

Once the FY2020 application is approved, EMIS will be provided an update file containing FY2019 program approval.

(This can only consist of industry credentials added for the 2018-2019 school year). A Senior Only Program should consist of a 12 point credential or a bundle of credentials that equal 12 points from the same career field.

Industry Credential Only Assurances

Approval of new Senior Only Credential Program applications or applications for renewal of an existing senior only credential pathway programs is contingent upon complying with the following assurances and that evidence of compliance is maintained on file.

1. Potential for students enrolled in the senior only credential program to receive the training providing the student to exit high school with a coherent bundle of credentials leading to meaningful employment or post-secondary options through any combination of credentials totaling 12 points within a single career-field.
2. An approved business/industry-recognized credential/s per the State Board of Education approved list is/are offered.
3. The program is in compliance with the guidelines of other state agencies as appropriate (e.g. Ohio Board of Nursing, Ohio State Board of Cosmetology, Ohio Department of Public Safety and State of Ohio Office of the Attorney General)
4. School administrators, teachers and business community members are committed to supporting the operation of the program as demonstrated through adequate planning, access to appropriate facilities, and commitment of financial resources and retention of qualified educators.
5. State CTE supplemental funds shall only be expended on costs associated with delivery of career-technical programming to CTE students for which the school district provides enrollment, performance and fiscal data to the Ohio Department of Education. OAC 3301-61-16 (B)
