Behavioral Health Navigation Referral Survey

Behavioral Health Navigation Services provides community members, providers, relatives or colleagues with information about resources available in Howard County. The Behavioral Health Navigator will then link the client to community resources and services and will follow-up on the outcome of the referral.

As the holiday season approaches, please be aware that response times to referrals may be longer than usual. To ensure efficient processing, kindly submit one form per individual and/or case.

The Bureau’s office hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., excluding State and Federal holidays.
