Behavioral Health Navigation Referral Survey

Behavioral Health Navigation Services provides clients, family members and providers with information about the resources available in Howard County. The Behavioral Health Navigator will then link the client to community resources and services and will follow-up on the outcome of the referral.

Please submit one form per individual and/or case. The Bureau's hours are 8am - 5pm,
Monday through Friday with the exception of state/federal holidays.
