Student Application 2017-2018

Student Hotspotting is an educational curriculum that facilitates teams of interprofessional students to learn about the challenges faced by complex patients during their interactions with the current healthcare system. Students from different health professions work together over a semester to learn how to identify "super-utilizers" and deepen the students' understanding of the factors that lead to high healthcare utilization and multiple readmissions. Students assist "super-utilizing" patients with care coordination and navigation. They also learn how to advocate for patients’ needs with providers and local social services.
 
Upon successful completion of the program, students will be eligible to receive a certificate of Excellence in Collaborative Practice, which will appear on their transcripts.
 
Timeline
·         Application: Due May 15

·         Capstone/Research Opportunities: Spring Semester
