Summer 2021 Concert Survey

Thanks for coming to one of our concerts in the park!

Filling out this survey helps the KSO gain information that ArtsWave and Kentucky Arts Council (funding agencies) require to support the KSO and to keep programs available free of charge to tri-state residents. Each completed form will be entered into a drawing in conjunction with each summer concert, and a drawing for the grand prize on September 10th. Winners will be announced on Facebook, after being contacted directly via email.

By completing it before noon on Tuesday, July 20th, you are automatically registered for two drawings:

1. $25 gift card drawing | Tuesday, July 20th
The winner will receive a $25 Walmart/Sam's Club gift card and drawing typically takes place a week after each summer program.

2. Grand Prize | Wednesday, September 10th
The winner will receive a KSO prize package worth over $475 including a $100 Amazon Gift card and a pair of tickets to each concert in the KSO’s 2021-22 Subscription Series. Drawing takes place Wednesday (September 10th) at noon. Be sure to complete the post-concert surveys for each concert to increase your chance of winning. Good luck!
