This application is for home-schooled students without a Statewide Student Identifier (SSID).

If a home-schooled student requests to take Ohio’s State Tests and the school or district allows the student to participate, please complete the following survey to apply for a unique ID from the Ohio Department of Education.

The home-schooled student's identifier is for test administration only. Do not include the home-school student's name. The district is responsible for keeping a record of each ID assigned to home-schooled students. Each home-schooled student should use the same ID for current and future test administrations.
