Hidden Treasures Honor Roll Enrollment Form

This form is designed to enroll one individual volunteer in the Hidden Treasures Honor Roll to be recognized at the Michigan Week Community Awards Breakfast. Leaders of Greater West Bloomfield organizations - businesses, clubs, faith community congregations, nonprofits, public agencies and schools - are invited to enroll a volunteer in the Hidden Treasures Honor Roll. This recognition is noncompetitive.

Please carefully review the information below and at the end of this document. Because all information must be completed in a single session, consider printing this document for planning purposes. Click on OK when completing each section. Thank you in advance!

AWARD ELIGIBILITY CRITERIA:
1. All enrollees must have lived, volunteered, attended school or worked in Greater West Bloomfield in the prior year. Only one volunteer in each organization will be recognized in this award category.
2. Recognition is based on the individual's voluntary and nonpartisan community service on behalf of the organization submitting this enrollment form.
3. Acts of service performed must benefit Michiganders or promote Michigan.

KEY ENROLLING ORGANIZATION RESPONSIBILITIES:  Each organization must do the following.
1. Answer each of the nine questions in this enrollment form. Responses are required to each question.
2. Click "DONE" after the last page to submit it. Provide additional follow up, if requested.
3. By March 2, 2018, email a digital photo of the honoree to DebMacon4@gmail.com.
4. By May 4, 2018, purchase a Breakfast Registration for the honoree and at least one additional organization representative.

KEY GREATER WEST BLOOMFIELD MICHIGAN WEEK AWARD COMMITTEE RESPONSIBILITIES:
1. Confirm enrollment is received, review eligibility and communicate award status to the organization.
2. Hidden Treasures will be  acknowledged in the printed awards program and their photos displayed when introduced at the Michigan Week Community Awards Breakfast on May 18.
3. Each Hidden Treasure receives a special medallion and certificate of recognition.
4. Invite elected officials serving Greater West Bloomfield to CONGRATULATE awardees.
5. Cablecast the awards program over Comcast channel 15 and AT&T channel 99, as well as make the program available OnDemand at www.civiccentertv.com and on YouTube.
6. Respond to questions emailed to the Awards Committee Chair. DebMacon4@gmail.com
7. Finalize award decisions.
