Introduction and Instructions

You may print this report out for your ease and convenience as you work through preparing the responses, but you must input your responses online by 10:00 PM May 2, 2016. If you need help or directions, please contact DGE Sue Goldsen at sue@goldsen.com WELL BEFORE the deadline.

You will find a copy to print at http://www.rotary6400.org/programs/districtawards.htm

You will note that the focus of many of the questions has changed toward an evaluation of club processes against best practice and the 6 focus areas. This is an intentional approach, the objective of which is to focus clubs on areas that Rotary International has deemed as important aspects of club governance and developmental areas of service to humankind.

There is no such thing as a perfect Rotary club. This is a self-directed questionnaire based upon the first statement of the Four Way Test (Is it the Truth?). A successful club will use this to constructively criticize and evaluate current processes and projects in order to improve service objectives to members, our primary customers, and in services to others. Please use this report as a way to benchmark your club's progress and as a basis to possibly develop a strategy improve your club's performance.
