Introduction

This questionnaire aims to evaluate the acceptability, effectiveness and implementation of Handover Bags for improving End of Life Care experiences of bereaved families.

Members of the community and health professionals aged 18 years or over are invited to participate in this questionnaire. You will require access to a computer, tablet device or a smartphone and the internet to complete this online questionnaire.

This questionnaire will take approximately 10 minutes to complete.

The questionnaire asks about whether you have seen or received a Handover Bag before, and your views of the acceptability and effectiveness of the bags. Handover Bags were developed by the Sunshine Coast Hospital and Health Service to return belongings of people who have died, to their family members. Please be aware that we have included simulated examples of clothing in a Handover Bag and a large plastic bag in the questionnaire.

Your views are valuable to us, whether you have experienced Handover Bags or not. We would greatly appreciate community members as well as health professionals completing the questionnaire so that we can understand a broad range of views and experiences. Your responses will assist us in understanding whether Handover Bags are appropriate bags to return belongings to family members and to improve End of Life Care.

Participation in this questionnaire is completely voluntary and your answers will be anonymous. The data from this study will be securely stored on a password protected electronic document by the associate investigator. We are conducting this questionnaire using SurveyMonkey®, which means that the information collected will be transferred outside Australia and stored securely on SurveyMonkey®'s servers. Results of this study will be compiled and written in a way that readers will not be able to identify participants.

Answering questions in relation to death and End of Life Care may remind you of a time when a family member or friend died. If you experience feelings of distress or discomfort when completing this questionnaire, please contact Lifeline on 13 11 14 or Beyond Blue on 1300 22 4636 in Australia, or your General Practitioner.

By completing and submitting this online questionnaire, you agree that:
  • You have read the above information
  • You voluntarily agree to participate
  • You are 18 year of age or older and are able to understand and give informed consent
  • Your responses will be securely transferred and stored on SurveyMonkey®'s servers
