Successful Handoffs for Touchdowns in Care Transitions Registration - April and May 2018

Attendee Information

There are four regional Successful Handoffs for Touchdowns in Care Transitions Conferences that will take place in April and May of 2018. Each event is the same, just in different locations to reach various regions of the state. You are encouraged to attend the program closest to your facility if possible.

CE credits are available to attendees of the events. Please choose the event you will be attending to register. Details regarding all four events, including parking information, will be available on the last page of registration. All events will take place from 1 - 5 p.m. local time.
