Consent to Participate:

We are currently engaged in a study of how often you use the Internet and  symptoms of anxiety and depression. To help us gain further insights into this area we will ask you to answer several questions.  There are both risks and benefits to participation.  Some questions may be about delicate personal matters such as your current emotional state, and you are free to stop participating at any time should the questions make you uncomfortable. The data you will provide will be recorded anonymously and your participation will be held in the strictest confidence.  You could benefit from this research by gaining increased insight into your mood and what impacts your mood.  It is important that you know that this study has been approved by the Research Ethics Review Board of Immaculata University.
We welcome questions about the experiment at any time. You may refuse to participate at any time without consequence or prejudice.
