Participants must complete the online survey available at CleanAirChoice.org. Incomplete surveys or surveys submitted by mail will not be accepted. Only one entry is allowed per household (determined by mailing address). Participants must be an Iowa resident with a valid Iowa address. Duplicated surveys will be disqualified. All surveys must be received by 12:00 p.m. on July 5, 2016 in order to be eligible.
