Welcome to this 'Agents for Change' Survey

Thank you very much for taking two minutes to complete this brief Agents for Change and Faculty of Medical Leadership and Management survey about speaking up for patient safety.

Recent inquiries from within NHS Trusts have revealed a culture where serious patient safety incidents were able to occur without being reported or adequately addressed. One of the concerns raised by these inquiries is that front line staff, including junior doctors, nurses and allied healthcare professionals, often do not feel empowered to speak up when they observe practices that compromise patient safety.

We would like to hear from junior doctors, nurses and other allied healthcare professionals about their experience of working on the front line and whether they are supported to speak up for patient safety.

The survey will be open until Friday 23rd October 2015. Results from this survey will be shared and discussed with attendees at the Agents for Change conference on Saturday 24th October 2015 and we will publicise the results online soon afterwards. Results will also be shared with the NHS England Professional Standards team.

This survey is anonymous and confidential. We ask that you do not provide any details that might identify patients or colleagues.
