COVID-19 Vaccination Status Self-Declaration |
As a research institute, PI is a strong believer in the science behind vaccinations and believes that a fully vaccinated community is imperative to the health and safety of its residents. We acknowledge that there may be exemptions for medical and religious reasons; inquiries regarding exemptions may be directed to People & Culture (peopleandculture@pitp.ca).
Anyone who attends PI will be required to complete this vaccination self-declaration prior to entry and before September 7, 2021, and resubmit if their vaccination status changes. Those who do not complete a declaration, are not vaccinated, or prefer not to disclose must undergo regular rapid antigen testing.
Testing is available onsite Tuesday (9 am-1 pm) and Thursday (10 am-2 pm) and offsite through non-PI programming. Testing must be completed within 72 hours in advance of OR upon entry to PI. Individuals are required to keep a record of the date, time, location, and result of their tests. In addition, everyone must continue to complete a COVID Screening Form prior to entry.
PI is committed to keeping your information completely confidential. In compliance with privacy legislation, access will be limited to only the necessary staff members immediately involved in administering this program/protocol. For the purpose of program administration and to ensure we attain maximum compliance, People & Culture will follow up with individuals who do not participate in the vaccination status declaration. Participation is mandatory to ensure continuous access to the building.
