Please note that this survey does not save progress and should be completed in one iteration for each course you would like to report.
For faculty reporting data for multiple courses: This survey is intended to collect data on one course at a time. Please fill out the survey once for every course you have SLO data to report. Each course requires it's own survey response.
In accordance with accreditation standards and Education Code, California Code of Regulations, Title 5; instructional programs shall establish regular review of curriculum. Completing this survey serves as a tool for instructional programs to document regular internal program review of curriculum and for programs to have access to this data.
