Healthwatch Worcestershire is an independent organisation that gives a voice to people who use health and social care services.

We want to know what young people think about mental health services for young people, especially the Child and Adolescent Mental Health Services (CAMHS) in Worcestershire.

We are interested to know how easy young people find it to access mental health support, how long they have to wait for support and how good they think the support is.

The following questions are annonymous. We would be very grateful if you could complete this survey if you or the young person you care for has accessed or tried to access CAMHS or support for mental health issues in the last 12 months.

We will use this information to give the people who run CAMHS feedback. This is epecially important as they are currently looking at what changes need to be made to CAMHS in the future.

Thank you for taking the time to fill this in.

If you are a parent or carer completing this on behalf of a child / young person, please answer the questions from their perspective.

* 1. Are you completing this survey about yourself or someone you care for?
