All skaters or, for skaters under 18 years old, their parents/legal guardians, are required to submit the following form within 24 hours of their lesson appointment until further notice.

Failure to submit the form 2 hours before your lesson appointment will result in your lesson being canceled.

Every skater is required to have a U.S. Figure Skating or Learn to Skate USA membership that is current and you must enter your membership number below. Membership ensures that you have sports accident insurance coverage. Please note that all previous memberships expired on June 30.   • Neither the rinks nor your coach are able to guarantee that you will not be exposed during your visit. You should evaluate your risk in determining whether to skate and take lessons.

  • I voluntarily assume all risks related to exposure to COVID-19 and the coach and the rinks disclaim any and all liability related thereto.

* 6. I agree that I and my skater will wear an acceptable face covering (mask) properly and socially distance from others, including the coach, by at least 6 feet at all times.

All acceptable face coverings (whether disposable or reusable) must:
  • Be made with at least two layers of breathable material.
  • Fully cover the nose and mouth and secure under the chin.
  • Fit snugly but comfortably against the side of the face.
  • Be secured with ties or ear loops and allow the skater to remain hands-free and not having to adjust the covering at any time.
At this time, based on guidance from health authorities, neck gaiters, open-chin triangle bandanas, and face coverings containing valves, mesh material or holes of any kind are not acceptable face coverings.

Medical masks, such as N95 or KN95, are not appropriate for athletic activity and should be reserved for health care workers.

You should supply your own acceptable face covering and also bring a back-up with you.

* 9. I understand that properly fitted equipment in good condition can help prevent injury and that my skater may have grown since the last time they skated.

  • I have checked the fit by removing the insole and making sure that the skater's feet are in the acceptable range for the size, as demonstrated in the photo below:

  • I have changed the laces in the boots.
  • There are not any loose screws and the blade does not wobble or move.
  • The blade is free of rust and has been professionally sharpened.
  • My skater hows how to tie their own skates properly.
  • If I had any doubts about sizing, fit, or condition, I have taken them to a reputable skate tech for inspection. The size and condition have been deemed satisfactory by a professional, reputable skate tech. 
  • I understand that my coach requires skaters to warm-up off-ice before entering the rink.
  • I understand that it is recommended that skaters put their skates on outside, before entering the rink and wear appropriate hard guards (Rockerz are recommended) to walk into the rink.
  • I understand that due to social distancing requirements, the coach is unable to check the skates or help skaters adjust or tie them.
  • I understand that most rink policies do not permit spectators and parents may be required to pay online ahead of the session, there will not be any refunds for cancellations or reschedules, and parents may not be permitted inside the rink.
