Informed Consent Form – Focus Groups

 

Your participation in a research project is requested.  The title of the study is “CULTURALLY RESPONSIVE FAITH FORMATION MODEL FOR THE CHILDREN OF LATINX MILLENNIALS AT DIVINE MERCY PARISH IN OAKLAND, CA.”.  The research is being conducted by Victor Valenzuela, a student in the Theology and Philosophy department at Barry University, and is seeking information that will be useful in the field of Theology and Culture.  The aims of the research are to find out if religious education programs in parishes has an effect on those Latinos who as adults choose to not identify with the Catholic Church.  In accordance with these aims, the following procedures will be used: A focus group interview.  We anticipate the number of participants to be 40 for the whole study. 

If you decide to participate in this research, you will be asked to do the following to participate in a 30-45 minute focus group

Your consent to be a research participant is strictly voluntary and should you decline to participate or should you choose to drop out at any time during the study you are free to do so without any repercussion. There are no known risks to you for involvement in this study.  Although there are no direct benefits to you, your participation in this study may help to further the understanding of how faith formation effects millennial Latinos and their children when it comes to living a faith filled life characterized by active participation in Church.

As a research participant, information you provide will be held in confidence to the extent permitted by law.  Although the researcher guarantees to keep all information obtained from the group confidential, due to the nature of groups confidentiality by group members cannot be guaranteed. All focus group members are asked to respect the privacy of other group members. Any published results of the research will refer to group averages only and no names will be used in the study.  Data will be kept in a locked file in the researcher's office All interview data will be destroyed after 5 years.
