Dear Parent,
Thank you for taking the time to respond to this survey, which is intended to capture feedback from parents who have experienced the death of their baby on a neonatal unit.
We are sorry for your loss and appreciate that responding to this survey may be difficult for you. If at any point you feel you need support, we have provided a list of organisations who could help.
Your responses to this survey will help us to continue improving care and services for bereaved parents on neonatal units by finding out "what parents need'" when coping with the death of their baby.
