Consent

The Follow Up Clinic is conducting a research project involving NICU families and personal wellbeing.  We invite you to complete this survey, “Supporting emotional wellbeing in the NICU and at home.” We are interested in learning from our parent community about what helped you cope with your NICU experience while you were in the hospital, and what has helped you cope at home.  We also want to learn about anything you’ve been doing to help support your children and their wellbeing. 

Your participation in this survey is completely voluntary and all of your responses are anonymous and will be kept confidential. We will be publishing results produced by the survey but none of the responses will be connected to identifying information.  There are no risks to participation and no direct benefit to you, although your responses will help other children and families.
