Ontario Health North East has a growing patient and family advisor engagement program. Completion of this survey indicates that you are interested in participating in our engagement program, which may include any of the following opportunities:
  • Joining the Ontario Health North East Patient and Family Advisory Council,
  • Involvement in various Ontario Health North East committees and projects that align with your interests and lived experience, and/or
  • Involvement in a patient engagement network that would enable you to participate in educational opportunities and future engagement opportunities.
This expression of interest survey will remain open to support our ongoing approach to recruitment. In support of our efforts to include diverse perspectives and experiences that better reflect the population of the North East region, there may be times that we are seeking to recruit individuals from specific geographical areas or seeking individuals with specific experience. If you are not selected for an interview at this time, with your consent we can keep your information on file for future opportunities.
