We commit to joining the Health Quality Innovation Network (HQIN) and being recognized for our commitment to improving population health.  Consultation services will begin in spring 2020, and last for about five years.  Participants may exit the HQIN project at any time by notifying KHC.  We understand the expectations of this project and have the support of practice leadership to:

  • Provide monthly data or reports to HQIN so we can monitor our performance.
  • Actively participate in virtual learning events, including webinars and sharing calls.
  • Improve chronic disease management, combat the opioid epidemic, avoid adverse drug events, and/or prevent C. diff.
  • Partner with other providers to improve health outcomes in our community.
