Health Quality Innovation Network

We commit to joining the Health Quality Innovation Network (HQIN) and being recognized for our commitment to improving population health. Consultation services will begin in spring 2020, and last for about five years. Participants may exit the HQIN project at any time by notifying KHC. We understand the expectations of this project and have the support of practice leadership to:
- Contribute to national quality improvement goals including reducing hospital readmissions, preventing C. diff, avoiding adverse drug events, combating the opioid epidemic, and eliminating disparities.
- Share and utilize aggregate data to monitor progress and performance.
- Actively participate in virtual learning events, including webinars and local meetings with other providers in our community.
- Partner with other providers to improve health outcomes in our community.
