Eastern Melbourne Primary Health Network is undertaking a co-design process to develop integrated AOD services to meet the needs of the EMPHN catchment. The co-design process is being led by 360Edge.

Three sources of information have been used to identify gaps and barriers including catchment data analysis, EMPHN needs analysis and co-design workshops.

This online survey is an opportunity to provide feedback on the outcome to date. The feedback will be collated and will guide the next stages of the development and integration.
