We are near the end or have completed a sport season.  We believe that your input is an important part in the process of improving our programs.  When completing this survey you may want to consider how the coach performed in the following areas in your response using the scoring below:

Highly Effective: Strong commitment to effective instruction that shows advanced expertise. The coach applies relevant instructional practices and is able to adapt them to players needs. These practices have a consistently positive impact on player performance.

Effective: Clear commitment to effective instruction. The coach applies relevant instructional practices that have a positive impact on player performance.

Minimally Effective: Initial commitment to effective instruction. The coach is using relevant instructional practices, but the practices need further refinement. With refinement, the impact on player performance can be increased.

Ineffective: Minimal or no commitment to effective instruction. Relevant practices are not being used or need reconsideration because they are not having their intended effects on player performance.
