Application Introduction:

Thank you for your interest in the HOPA/ASCO Quality Training Program (QTP) 6-Month. The QTP is a transformative 6-month journey that combines in-person and hands-on learning to enhance healthcare quality. Over five days split across three in-person Learning Sessions, participants will engage in seminars, case studies, and small group exercises, culminating in hands-on projects at their individual practice sites. This will be a thematic program, with all teams required to work on projects related to precision oncology.

Application Preparation:
The Team Lead should submit the application on behalf of the whole team. Before beginning your application, please ensure you are prepared with the following:
  • Name, email address, and phone number for all team members
  • The CVs of all team members
  • A signed statement of support from your leadership (TEMPLATE)

Application Steps:
  1. Team Lead & Member Information: Start by entering details for the Team Lead (Team Member #1) and up to three additional team members. This includes personal information, credentials, job title/position, and a CV upload.
  2. Practice/Institution Information: Provide comprehensive details about your practice/institution, including name, address, contact details, and a brief description of your patient population and organizational structure.
  3. Project Proposal: Outline what your team hopes to learn through the QTP and describe how you hope to improve quality of care delivery with your project. As a reminder, the problem you are trying to address should fit within the theme of Precision Oncology.
  4. Support & Commitment: Upload a statement of support from your institution's leadership, reaffirming commitment to the quality improvement project and the time necessary for participation in the QTP.
  5. Program Fees, Policies, & Application Submission: Review and agree to the program's terms, including responsibilities for fees, reporting, and publication.
Final Notes & Submission:
After reviewing the program requirements and policies and indicating your agreement, you're ready to submit your application.
