Proposal to present at the 12 Annual HODS Symposium

Thank you for submitting your proposal to present at the 12th Annual HODS Symposium. Please respond to the following questions. By submitting this proposal, you understand that your presentation will be recorded and that with your submission, you automatically permit HODS to publish the title, description, PowerPoint, and presentation recording to any media outlet HODS chooses.

3.Title of your presentation, including your name and credentials as they will appear at the program. (Make sure you include any HODS credentials you have such as D-EDX, R-EDX, FMSK etc)
4.Brief description of your presentation as it will appear in the program. No need for official abstract, but if available, you can use that instead of the description. (200 words maximum)
5.Presentation Learning Objectives (4 maximum)
6.Content outline. Provide briefly the sequence of the items you will be presenting.
7.Bibliographic References for Clinical Presentations (5 to 7, after 2018)
8.How many minutes of your presentation does your content include differential diagnosis components. This is for clinical presentations.
9.Please provide a brief bio about yourself; the kind of work you do; any degrees, certifications, achievements; interests, and anything else you would like to appear about you in the event program.
