This form is for homelessness service to register to participate in the Client Satisfaction Survey 2023.

Survey collection opens on Monday 3 April and closes on Friday 28 April 2023.

2.Are you registering an organisation, service program or individual site?(Required.)
3.If you are registering for a site only, please name the site followed by the service program and organisation it operates under.
4.If you are registering a service program only, please name the service program followed by the organisation it operates under.
5.If you are registering an entire organisation only, please name the organisation.
6.If your organisation has participated in the Survey in previous years and has changed name since then, please indicate the former name so we can make sure to include the year-on-year data for your service in your report.
