THIS QUESTIONNAIRE IS FOR LIBRARIES OR OTHER DEPARTMENTS IN PRISONS OR YOUNG OFFENDER INSTITUTIONS WHO HAVE BEEN RUNNING THE SIX BOOK CHALLENGE. IT SHOULD BE COMPLETED BY ONE PERSON FOR EACH PRISON OR YOUNG OFFENDERS INSTITUTION.

There are separate evaluation forms to be completed by public libraries, learning providers and workplaces running the Six Book Challenge 2010.

Please look through the whole form before starting to complete it as you will need a range of statistics to hand. It may be easier for you to print it off to complete the statistics section before you start filling in the form online.

You can save your work on the form by clicking the appropriate button at the bottom of each page and then return to it later.

Please be sure to complete this questionnaire by WEDNESDAY 7 JULY 2010 at the latest so that we include your feedback in our analysis, even if you are still running this year's Challenge. The detail you provide will inform our development of the Challenge.

All completed forms received by WEDNESDAY 7 JULY will be put into a prize draw for new books received from publishers through our Reading Partners scheme
