[2021] New Zealand Head and Neck Cancer Survivor's Support Network - Patient+ Annual Survey

Navigating the healthcare system has never been more complex, and no one gets a clear roadmap. For those of us affected by head and neck cancer the journey is hard.

Whatever your story, it matters. You matter.

That is why we are asking you to share your healthcare experience. We are here to listen, learn, and educate others about your opinions, experiences, and more.

We thank you for taking the time to complete our brief survey and contributing your voice to a community focused on living their healthiest lives, in whatever shape that may take.

The survey is prepared by the Head and Neck Cancer Support Network (http://headandneck.org.nz).

The Patient+ Annual Survey should take approximately 10-15 minutes to fill out (depending on your responses) and should be completed by the end of 5pm 15 August 2021.

Completing the questionnaire indicates your consent to participate in this project.

We have called this questionnaire Patients+. We want to hear from more than patients. We invite you to share this questionnaire with your carers, family/whānau, and friends. We encourage you to share your experiences and opinions to ensure we can advocate for you.

Please use the "Give comments" sections to share your experiences. 

No computer Internet Provider (IP) addresses will be collected via www.surveymonkey.com (i.e. the address of the computer you use to complete the questionnaire). You can complete this questionnaire only once.  We value and respect your privacy. Your responses cannot be identified. All the collected data from the questionnaire will be kept for five years in a secure location. A summary of the questionnaire results will be available at http://headandneck.org.nz/survey-results at the end of August.
