Welcome to the Choice Pass Youth Survey!
This survey is from the Gunnison County Substance Abuse Prevention Project (GCSAPP). We use it to learn what young people think about alcohol, drugs, and the Choice Pass program. The survey takes about 5–10 minutes to complete. Choice Pass is a voluntary program; you do not have to join. If you choose to take part in Choice Pass, completing this survey is a required part of the program.
Your answers will be kept private. They will be stored securely, and only project staff will see them. We will never share your name or any information that could identify you when we report the results. Thank you for taking the time to complete this survey.
