This survey is a part of a collaborative effort between Cook County Public Health and Human Services, Sawtooth Mountain Clinic, ISD 166, and a pilot project called Rural Mamas Connect (RMC).  RMC is an online platform for pregnant people living in rural areas to connect with one another and learn about pregnancy, birth, and early parenting. 

The purpose of this survey is two-fold:  
· To better understand the needs and experiences of people who are pregnant, birthing, and/or raising a small child while living in Cook County 
· To guide the content and approach of Rural Mamas Connect so that the platform can be as helpful and relevant as possible to the families it will serve 

We are looking for people to respond who are or have been pregnant in the past three years, and who are living in Cook County. 

Participation in this survey is entirely voluntary.  You are free to answer any, all, or none of the questions on this survey.  The benefits of completing this survey include providing information that could benefit people who are pregnant, birthing, and raising small children in rural communities.  There is no monetary or material benefit to individuals for completing this survey.  The risks to participating are minimal, but if you experienced trauma, depression, or anxiety in your pregnancy, birth, or postpartum period—survey questions may bring those feelings to the surface.  Please take care of yourself and reach out for help as needed.
