Children & Health Committee - Public Input

Introduction

The Ohio Developmental Disabilities Council welcomes public feedback on its 5-Year Plan on Developmental Disabilities. This survey will give you a chance to submit your own comments on each of the projects being offered by the Children & Health Committee of the Council.

We will not ask for your name or any other information that could be used to identify you. This information is being provided in a confidential manner and will only be used to help the DD Council develop their next 5-Year State Plan on Developmental Disabilities.
