Healthy Kansas Hospitals Centers of Excellence
Healthy Kansas Hospitals Centers of Excellence Award recognizes outstanding and innovative hospitals who have made formal changes (through a new policy) as part of participation in Healthy Kansas Hospitals.
HOW TO COMPLETE THE NOMINATION FORM
Please note - at any time you may close the browser to return later. The software will save your responses. When you return, you will go to the next question to complete the rest of the survey. You will need the hospital's contact information as well as descriptions of how the nominated hospital meets each of the following criteria:
Impact - makes a visible difference in improving the health and wellness of the hospital through participation in Healthy Kansas Hospitals.
Innovation - takes a new, creative approach and/ or addressed an emerging challenge or need related to participation in Healthy Kansas Hospitals.
Leadership - leadership team takes a proactive and supportive role in the implementation of the changes with participation in Healthy Kansas Hospitals.
Partnership - works closely with local community, county, state and/ or partners to improve the health of the community/hospital.
Sustainability - the hospital has taken steps to sustain the changes as the result of participation in Healthy Kansas Hospitals.
THE PROCESS
Nominations are submitted online and are reviewed by a committee of partners familiar with healthy food efforts. The committee does not include Kansas hospital executives but does include previous Centers of Excellence Award Winners. The committee may recommend up to four awards be given. The committee’s recommendation will be presented to the KHERF Board for approval.
DEADLINE
Nominations will be accepted until 11:59 p.m. on Friday, February 3. You will receive an email confirmation that KHA has received your nomination. If you do not receive a confirmation by February 6, please contact KHA at (785) 233-7436.
AWARD PRESENTATION AND RECOGNITION
In recognition for an outstanding and innovative contribution to Healthy Kansas Hospitals the awarded hospital will be honored by:
• Awardees will be recognized in a media/award presentation in their community, in a statewide media release, touting efforts to local media.
• KHA/KHERF will recognize the impressive commitment made to provide healthier food (or beverages or education) to your staff, patients and community by coming on-site during March or April to present the actual award.
• Cash contribution of $1,000 to your food service, education or wellness program.
