For years, the Ohio Association of Free Clinics had recognized the amazing care that free and charitable clinics have provided, but found that there were no quality standards for free clinics to quantify the level of care. They set to work adapting the NCQA standards for Patient Centered Medical Homes for free clinic use, and in late 2015, OAFC rolled out the first version of the Free Clinic Membership Quality Care Standards.

Since the first time standards were released, we have been working with Jim Benedict, PhD, PT and Ken Learman, PhD, PT from Youngstown State University to analyze the data provided. However, as we started working with the data, we realized that there are a few questions that needed clarification as well as an adjusted scoring system.
This electronic survey includes these changes and updates, and provides an easier, streamlined data collection procedure. Each page features a different element to the quality standards. At the top of the page, you will find an "Explanation and Documentation" paragraph.  This is meant to help explain each question.  One major adjustment made to this survey from the last is that each question is weighted equally and is answered "yes" or "no".
