HICCUP Pre-Test

Thank you for registering for Hi-Conflict Co-Parenting UnPlugged (HICCUP).  The following form must be completed prior to the start of the HICCUP class.  Failure to complete this form, prior to the start of the class, will result in your inability to attend the scheduled training course.
