Hope and Health Community Collaboration Expression of Interest Survey

Introduction

Thank you for your interest to bring the Hope and Health magic to your community! This looks different in every situation - so we created this EOI form to gather some preliminary information so we understand your needs, resources and vision.

The next step will be to set up a phone/zoom call to collaborate and co-finalize the details should there be mutual interest to move forward.
