This survey has been created by the European Forum for Primary Care in order to ask our members to respond to some crucial questions on aspects of Primary Care Organisation, in relation to the COVID-19 crisis. 

The survey has been divided into two sections:

Part 1 - Process and Guidance 

Part 2 - Experience and Observations

We would like to ask you to complete these forms to the best of your knowledge in order to gain a complete picture on how Europe is responding, as well as how we can learn from one another's insights/experiences.
