You are being asked to complete this survey because your child(ren) participates in the CareerVisions New York afterschool program. Participation in the survey is entirely voluntary; however, we hope you will choose to complete the survey, as your responses will be very helpful in providing us with an understanding of the successes and challenges of the program and help inform future enhancements.
Only staff from the evaluation team will have access to your survey responses and will keep your individual responses strictly confidential. Please note that you can skip any question on the survey if you wish. The survey should take about 10 minutes to complete.
