Seeking Volunteer Test Sites

MN Community Measurement (MNCM) is seeking volunteers to participate in the pilot testing of a new patient reported outcome (PRO) measure for patients with chronic obstructive pulmonary disease (COPD), "Controlling the Impact of COPD on Health Status." This measure was developed by an MNCM-convened, multi-stakeholder COPD Measure Development Workgroup, which completed the development of measure specifications in November, 2016.

The purpose of this survey is to assess the level of interest and suitability of potential test sites for participation in the upcoming pilot testing of this measure.  Please refer to the corresponding communication for more details, including the timeline and measure specifications.
