Applicant Name, complete address with city and zip code, cell phone number. Number of people in the household. Is anyone in the family a US veteran or currently on active duty? Please indicate who on the question below. Please include a narrative as to why our donors should pick your family. A few sentences will be sufficient. We will communicate via text message if you are adpoted and we receive presents for your family.

Please give the following information for all Adults over the age of 18 in the household. Adults must be living in the home. Name, gender, clothing size, shoe size, needs, wants, and a big wish. Please be complete and specific. Make sure to specify if the sizes are mens or womens.

Please give the following information for all chilren aged 18 and younger living in the home. Name, age, gender, clothing size, show size, needs, wants, and a big wish. When giving clothing and shoe sizes please specify if the size number is toddler, child, or adult and boys or girls. Sizes for children often range very drastically and we want to make certain they are correct for everyone. Please make sure all information is complete.

Do you have any pets, if so and you would like gifts for your pets? Please list the type of pet and number of them you have. we will try and accommodate. If the animal is a service animal, please indicate that as well.

I understand that Adopt a Family operates on donors and gifts are not guaranteed. I also understand that I must answer all questions completely in order to be approved. I also acknowledge that donors will NOT be given personal identifiers such as name, address, or contact information and that I will not receive such information about my donor if chosen. I also know I am responsible to pick up the gifts if I am notified I have them. I have the ability to pick up in Live Oak. Delivery is not availale.
