This survey collects information about mental health, substance use, and services in your community. It includes questions for both adults and youth. Your answers will help improve programs and services in your community.
The survey is anonymous, takes approximately 5 minutes to complete, and is recommended for youth ages 12 and older (or those who are middle school-aged and above).
Complete the entire survey to be eligible for the gift card drawing. After submitting your completed survey, you'll find a separate link where you can voluntarily provide your contact information to be entered into the drawing. Your contact information is collected separately and cannot be linked to your survey responses, ensuring your survey remains anonymous.
