Purpose: This survey is designed to reflect the degree of how we connect, respond, and support each other within our communities. We define our community as members who identify with the Black Diaspora and all its intersections (ex. Queer, mixed race, refugee, immigrants, multi-generational, etc.). We offer sincere gratitude to HEAAL Coalition members and staff; South Mountain W.O.R.K.S.; and Circles of Peace in creation and development of the survey. This was truly a labor of collaboration and love.
This survey is completely voluntary and anonymous. That means you can choose to answer as many or as few questions as you want, and we will only be looking at all of the data from everyone together and not your specific individual responses.
