Teen Volunteer Interest Form

Basic Information

Thank you for your interest in volunteering at the Library! Please fill out the following form to the best of your ability. The Library will use this information to contact you if you are selected as a potential candidate for a volunteer position.

Note: Teen library volunteer openings are limited. The Library will keep your Interest Form on file for three (3) months from the date of submission. If you have not been contacted or considered after that period of time, please feel free to resubmit your Interest Form.

Approval of all applicants will be at the discretion of the Library Volunteer Resource Managers. The Library needs not to specify a reason for unapproved applicants.
