Thank you for being a part of UT's Human Dimensions of Organizations' seminar, Risky Business. I'm looking forward to meeting you on October 21st. We'll have time to meet each other from 8:30-9:00 AM, and the class runs from 9:00 AM - 4:00 PM.

If you don't mind, please take a few minutes to fill out this brief survey. It will help me to build the materials to better fit your needs, and give a view into your perspectives on human error. We'll start the class with an introduction, and show the statistics and word clouds built from your responses. Please make sure to keep your responses anonymous.

I'm looking forward to meeting you!
