Dean/President Change Form

This form is designed for CAAHEP-accredited programs to submit information for any changes in either President or Dean (The "Dean" contact is the Medical Assisting Program's Immediate Supervisor).

If your change is not related to the President/CEO position or the immediate supervisor of the Program Director, then there is no need to report it to us.

This form must be submitted for processing by MAERB for current personnel to be correctly identified in both the MAERB database and the CAAHEP database. Please allow for several business days for any additions/revisions to be made. For any questions about this form, please contact MAERB@maerb.org
